Staying Healthy With Possible Nationwide Quademic: Flu, RSV, COVID, Norovirus

As we move through the late winter months, respiratory and gastrointestinal illnesses continue to rise across the country. The Centers for Disease Control and Prevention (CDC) has reported that flu levels are at their highest since the 2009 swine flu pandemic. 

At the same time, COVID-19, respiratory syncytial virus (RSV), and norovirus are spreading across the country, creating what some experts are calling a “quademic.” These four viruses are affecting communities in different ways, but the risk of illness is increasing nationwide.

It is crucial to understand how these viruses spread and take steps to protect against them. Preventative healthcare measures, such as vaccinations and proper hygiene, play a significant role in reducing the severity of illness.

Flu Activity at Its Highest in 15 Years

The flu is hitting hard this year, reaching levels not seen since the peak of the 2009 swine flu pandemic. The CDC reports that nearly 8 percent of outpatient visits this season have been related to influenza. This is the highest percentage recorded in the CDC’s influenza surveillance network in more than a decade​.

Many states are seeing “high” or “very high” levels of flu activity, with infections continuing to rise in at least 15 states. Emergency room visits and testing laboratories are also seeing unusually high numbers of flu cases. Flu test positivity has reached 31.6 percent, nearly doubling the previous season’s peak of 18.2 percent.

Unfortunately, flu vaccination rates are lower than in previous years. The CDC reports that flu vaccination rates among children are at their lowest level in six years, and fewer than half of all Americans have received the flu shot​. 

Since flu can lead to complications such as pneumonia, severe dehydration, and even neurological issues, getting vaccinated remains one of the best ways to reduce the risk of severe illness.

COVID-19 Cases Remain a Concern

While the country is no longer experiencing the massive surges seen in past winters, COVID-19 is still a significant public health concern. As of January 2025, the national test positivity rate was 5.4 percent. COVID-19 accounted for 1.6 percent of all U.S. deaths that week​.

Despite the continued circulation of the virus, vaccine uptake remains low. Only 21.5 percent of adults and 10.6 percent of children have received the updated COVID-19 vaccine​. Many experts warn that cases could increase further in the coming weeks, following the peaks of flu and RSV. Since immunity from previous infections and vaccinations declines over time, staying up to date on vaccinations is essential.

For individuals with underlying health conditions or weakened immune systems, COVID-19 can still pose a serious risk. Primary care providers can help patients understand their personal risk levels and offer guidance on available treatment options.

RSV Cases Declining but Still Present

RSV primarily affects young children and older adults, and it can cause severe respiratory complications. This virus typically peaks in December and January, and recent reports suggest that cases are now declining​.

RSV infections are common, with nearly every child contracting the virus by the age of five. However, the virus can be particularly dangerous for infants under one year old, older adults, and individuals with chronic conditions​.

Vaccination options for RSV have improved in recent years, but uptake remains low. Only 43.6 percent of adults over 75 and 33.6 percent of adults between 60 and 74 have received the RSV vaccine​. Staying informed about vaccination options and consulting with a primary care provider can help reduce the risks associated with RSV.

Norovirus Cases Are Increasing

Unlike flu, COVID-19, and RSV, norovirus is a gastrointestinal illness rather than a respiratory infection. It spreads easily in enclosed environments and is known for causing outbreaks in schools, cruise ships, and other crowded spaces.

Recent data shows that norovirus cases have been increasing since early fall, with test positivity reaching 22.6 percent in late December​. This highly contagious virus can survive on surfaces for long periods and is resistant to many common disinfectants​.

Since norovirus does not have a vaccine, prevention strategies focus on hygiene. Handwashing with soap and water is the most effective way to prevent infection, as alcohol-based hand sanitizers are not effective against norovirus​. If symptoms such as vomiting and diarrhea develop, staying hydrated with fluids that contain electrolytes is crucial.

What Can Be Done to Stay Healthy?

With all four of these viruses spreading at the same time, taking proactive steps to reduce the risk of infection is more important than ever. Experts emphasize that vaccination is a key component of prevention. Flu, COVID-19, and RSV vaccines are widely available and can reduce the severity of illness, even if infection occurs​.

Practicing good hygiene habits can also help prevent illness. Washing hands frequently, covering coughs and sneezes, and avoiding close contact with sick individuals can lower the chances of infection. Since norovirus can spread through contaminated surfaces, cleaning frequently touched areas with diluted bleach is recommended​.

Staying informed about local virus activity can also help individuals make safer decisions about gatherings and travel. Checking flu and COVID-19 case numbers in the area and being mindful of symptoms can reduce the spread of these viruses.
